Supplemental Documentation

After submission of the CME accreditation application, additional documentation will need to be submitted to UC San Diego CME throughout the planning process and prior to the start of the activity for review and approval.

Marketing Materials:
UC San Diego CME must approve all marketing materials prior to printing/distribution. A guide to marketing material requirements can be found on our planner tools page. Please allowa minimum of three business days for review.

Letters of Agreement for Commercial Support (LOAs):
UC San Diego CME must review and sign all LOAs prior to the start of the activity. Note that in agreement with ACCME standards, all LOAs must be issued to the accredited provider. For more information visit our FAQ section. View a UC San Diego LOA.

Outstanding Disclosure Forms:
All outstanding disclosures are required to be submitted via the online disclosure database no later than four weeks prior to the start of the activity. This timeline is required in order for us to review and to resolve all conflicts of interest prior to the start of the activity.

Syllabus/Handouts:
UC San Diego CME must approve all syllabus material and other handouts prior to printing. A guide to syllabus/handout material requirements can be found on our planners & coordinators tools page.

Activity Materials (enduring activities only):
UC San Diego CME must review and approve the content of all enduring activities prior to printing/distribution. Guidelines can be found on our tools page.

Sign-In/Out & Certificate Templates:
Participants must sign to verify that they participated in the activity and claim the number of credits. Our sign-in/out sheet template can be found here. After signing out, attendees shouldl receive a certificate of credit for participating in the activity. Certificate templates for MD's and non-MD's along with instructions can be found here.

Evaluations:
CME activities must be evaluated according to their effectiveness in achieving the learning objectives/desired outcome and for their projected/actual impact on clinical practice. UC San Diego CME utilizes two different evaluation forms, one for activity evaluation and one for speaker evaluation which should be used unless an exception is requested prior to the start of the activity.
