Exhibit Fee of $2,000 Includes:

• 6' table top exhibit
• Company support acknowledgment on website, printed materials & special recognition at the meeting
• Up to 2 representatives to attend

Please print a copy of the exhibitor form and fax the completed form to (619) 543-7610, or e-mail to cfeatherstone@ucsd.edu at your earliest convenience. Exhibit space is limited and this form reserves your spot. Exhibit space checks should be made payable to UC Regents. Further exhibit information will be sent as we near the conference date.

Grant Support Levels:

• Platinum: $25,000 and up
• Gold: $10,000 - $24,999
• Silver: $5,000 - $9,999
• Bronze: $4,999 and below
