*For Residents and Fellows: Application must be accompanied by a letter from the Chief of Service stating that the participant is enrolled in a full-time approved training program (Residents and Fellows).
The course syllabus will be distributed as a downloadable .pdf file. A link to the file will be e-mailed to you prior to the meeting.
*You may purchase a printed syllabus prior to March 21, 2014 for an additional $100. If purchase is made before March 3, 2014 your pre-ordered syllabus will be available when you check into the conference.
The syllabus as well as any missing presentations will be available for download on the conference website approximately (1) one week after the conference.

Cancellation Policy: The full course tuition is refundable, less $100 if your cancelation is received in writing, on or before March 24, 2014. No refunds will be allowed after this date.
In the unlikely event that the course is canceled, the Weil Institute and UC San Diego are responsible only for a full refund of the registration fee and not for transportation, hotel accommodations or any miscellaneous expenses.
